Physical Exam Information

There are three categories for required physical examinations

       Enrollment Physicals

Each student must have documentation of a physical exam within 30 days of enrollment.  This is a NJ state law.  Click on the link below to access the Universal Child Health Record.  Click here for the required document for your physician or nurse practitioner to complete.

        Interscholastic Sports Physicals

All students wishing to tryout and/or participate in an interscholastic or intramural sport in the state of New Jersey must have the following: 

-        a pre-participation health history completed by the parent/guardian

-        a pre-participation physical exam done by your private doctor within 365 days of the first practice/tryout session.  The school physician is available for physical exams in June and August (see schedule in May of each year).

-        A pre-participation consent to participate in InterScholastic sports AND to permit random urine drug screenings and NJSIAA required random steriod testing

-        If your child has been medically cleared, all of the above paperwork has been submitted and permissions are signed BUT it has been more than 60 days between the physical exam and the start of the sport season, a 60 day health history update is required.  This must be completed and signed by the parent/guardian

Below are the links to the required forms.  The forms are State of NJ / Department of Education forms and we are required to use these forms ONLY.  Documentation from your private physician that is not on this form will not be accepted.  Also, be sure that your physician completes the form entirely or your child will not be found eligible to participate pending the missing information.  A signed parental permission/sports consent form is also required.

     Periodic Physical Exam recommendations

Each student in Grades 4 and 8 should have an updated physical exam for the their student health record.  This is not a requirement but a recommendation. Parents will receive a letter advising of this recommendation.  If you choose to have a physical done, click on this link and have your doctor complete the form.
